Entries will only be accepted through appointments made with our clinic by January 26, 2024. Winner must be 18 years of age or older and may be required to sign a release. Winner must agree to pick up their prize at the clinic at which they were chosen winner. Prize will not be mailed. Employees of the practice and immediate family members are not eligible to participate. Winner may not request substitution or other consideration from the originally awarded prize. These rules are subject to change without notice. Any prize not claimed within 30-days will be forfeited.
